In coordination with the Rotary Club of Bacolod, Mayor Evelio Leonardia
announces the launching of this year’s “Search for Outstanding
Employees” to give cognizance to the deserving employees who unselfishly
and selflessly dedicated their time, talent and ability in the name of
public service. This is in line with the 66th Charter Anniversary of
Bacolod City on October 19, 2004.
Guidelines for the selection of the Outstanding
Employees include Performance - 50%, Personality Traits -25%, Community
Service - 25%.
Employees in the career service who holds first
and second grade positions with at least three years in service, have not
been found guilty of any criminal or administrative offense or does not
have any pending case at the time of nomination and have not been
previously awarded as “Outstanding Employee” may be nominated by his
immediate supervisor or his department/office head.
The nomination shall be endorsed by the majority vote of
co-employees/co-workers, other NGO or any private person can also nominate
any qualified employee for the said title.
A nominee shall pass the following requirements:
formal letter of nomination, certification of nominees to have meet the
criteria set for the Selection of the Most Outstanding Employees Award,
Personal Data Sheet (CS Form 212), Certification of the Department/Office
Head, supporting documents such as clippings, pictures etc., Statement of
nominee’s duties and responsibilities in prescribed form (PDS),
Performance Evaluation Sheet (PES) Report for the two rating periods; July
to December 2003 and January to June 2004, Two (2) copies of 5 cm X 5 cm
photo, Updated Service Record and referrals from school, residence
(neighbors), or co-employees.
All nominations together with supporting
documents shall be submitted to the Rotary Club of Bacolod not later than
October 1, 2004, thru the Human Resource Management Services, 4th floor,
Bacolod City Hall.
The city mayor said the awards for the selection
of Outstanding City Employees shall be given in fitting ceremonies on
October 19, 2004.
