CHECKLIST OF REQUIREMENTS FOR 
AVAILMENT OF INCENTIVES

1. Three (3) copies of duly accomplished application form, duly notarized supplied by the Center.
2. A copy of the project study of the proposed investment Form supplied by the Center.
3. A certified true copy of its Certificate of Business Name Registration from the  Department of Trade and Industry.
4. Certificate of Registration if registered under the national law (EO 226 RA 7844,RA 7916,RA 7718)
5. Certificate of compliance requirements under existing national / local laws by accredity national / local agencies.

1. Item No. 1,2 4 and 5 under Single Proprietorship 
2. A copy of the company's Articles of partnership / Incorporation and By - Laws as approved by the Security and Exchange Commission.
3. Authority from the partner or the Board of Director as the case may be to file the application.
4. Certificate of Compliance.

2. A copy of the Certificate of Registration from CDA
3. Authority from the Board of Director to file application
